On November 28, 2006, The Madison County Health Department was selected as one of six sites in the state to establish “Diabetes Centers of Excellence” that will target the Medicaid population in counties they serve. This program helps Medicaid members and others with diabetes better manage their disease through a unique partnership involving the patient, primary care physician, local health department and community resources.

The Center is staffed by two registered nurses and a registered dietitian who are certified diabetes educators, a registered nurse and an administrative assistant. The Center is available by telephone at (859) 623-3462 or by email at DCOE@madisoncountyhealthdept.org.
